Health Requirements

Proof of Immunizations

All students are required to show proof of immunizations and adhere to the most current requirements regardless of age.

Students in K – 8 must have the following immunizations prior to the first day of school.

• 4 POLIO
• 5 DTP
• 2 MMR
• 3 HEPATITIS B
• 2 VARICELLA
  
  

*Students entering 7th Grade must have documented proof of a Pertussis (Whooping Cough) Booster Immunization (Tdap) given after the age of 7. This is a CALIFORNIA STATE requirement and must be met with documentation turned in prior to the first day of the school year.
